Job Overview:
Livingston Healthcare is seeking an experienced Case Manager RN for a 13-week contract to support case management on the Medical/Surgical floor. This role focuses on discharge planning and utilization review to ensure optimal patient care, resource management, and regulatory compliance. Work hours are Monday through Friday (occasional weekend coverage may be required), within standard daytime hours (8 AM–5 PM).
Key Responsibilities:
Perform admission, concurrent, and retrospective utilization reviews
Facilitate appropriate admissions and discharges in coordination with physicians and leadership
Conduct thorough discharge planning assessments with patients and families
Coordinate referrals to skilled nursing facilities, rehab centers, or specialty care hospitals
Maintain strong communication with interdisciplinary teams and community services
Participate in quality improvement, patient satisfaction, and readmission reduction initiatives
Review medical records for compliance with medical necessity and intensity of service
Lead or contribute to utilization review meetings and reporting
Requirements:
Active RN license (Montana or Compact)
Experience in case management, discharge planning, or utilization review required
Strong understanding of CMS standards and regulatory compliance for UR and discharge
Excellent communication and coordination skills
Ability to work independently within a multidisciplinary team
