Job Title: Home Health Nurse
Facility: Elara Caring Home Health – Elkhart
Location: Elkhart, Indiana (serving Elkhart & LaGrange Counties)
Shift: 8-Hour Day Shifts
Guaranteed Hours: 40

Role Overview:
Provide skilled nursing care for patients in the Elkhart and LaGrange County areas. Responsible for patient visits, Start of Care (SOC), Resumption of Care (ROC), and recertifications (Recerts).

Key Duties & Responsibilities:

• Conduct patient visits and assessments

• Complete SOC, ROC, and recertification documentation

• Collaborate with interdisciplinary team to ensure high-quality care

Required System Knowledge:

• OASIS

• HCHB (Home Care Homebase)

Orientation:

• Must attend an orientation session with the Talent team at the Elkhart branch on either Monday or Wednesday
