Schedule: Day Shift | Full-Time (35–40 hours/week)

Role Brief

Optimae Home Health Services is seeking an experienced Home Health RN to manage a weekly caseload of approximately 27 visits, covering a full range of home health services. This role supports patients across the Burlington area and requires strong OASIS and field experience.

Scope of Work

• Conduct admissions, recertifications, and resumption of care visits
• Perform follow-ups and discharge planning
• Manage documentation using Carevoyant EMR
• Coordinate care to ensure patient outcomes and continuity

Candidate Profile

• Active Iowa RN License
• 2+ years recent Home Health experience
• Strong OASIS experience required
• BCLS certification
• Comfortable managing independent field visits

• 2-step TB
• Immunizations: Hep B, MMR, Tdap, Varicella
