Position Overview
BAYADA Home Health is seeking an experienced Home Health professional to join their Montgomery County team. This role focuses on delivering high-quality, patient-centered care in a home setting while building strong relationships with patients and families.
Key Responsibilities
Provide skilled home health services in accordance with the plan of care
Perform patient assessments and ongoing evaluations in the home environment
Educate patients and caregivers on care plans and health management
Maintain accurate and timely documentation
Coordinate care with the interdisciplinary team
Requirements
Previous Home Health experience (required)
Ability to work independently in a home-based care setting
