Position Overview:
Schoolcraft Memorial Hospital is seeking a dedicated Home Health RN to support our HomeCare and Hospice programs during an EMR transition. While hospice experience is preferred, we will consider candidates with strong home health experience who are open to occasional hospice coverage.
Key Responsibilities:
Provide skilled nursing care across acute and chronic conditions (post-surgical, geriatric; minimal pediatric).
Complete 5–6 routine patient visits per 10-hour day.
Travel within service area (approx. 1-hour radius from office); routes grouped for efficiency.
Participate in on-call rotation (about 7 days/month, every 3rd–4th weekend, 8 AM–8 AM coverage).
Document patient encounters using Home Health Notify; transition to MatrixCare EMR (Hospice: Oct 1, HomeCare: Oct 13).
Ensure compliance with HomeCare and Hospice care standards.
Requirements:
Active RN license (Michigan).
BLS Certification.
OASIS experience required.
Previous skilled Home Health experience required; Hospice experience preferred.
Valid driver’s license and reliable transportation.
Additional Details:
Orientation includes EMR training, policy/procedure review, and shadowing with staff RNs.
12-week assignment.
Facility provides support during EMR transition.
