Position Overview

UP Health System Home Care & Hospice in Marquette is urgently seeking a Home Health Therapist to provide in-home rehabilitation services. This is a short-term, full-time assignment with consistent day shifts.

Key Responsibilities

• Deliver rehabilitation care to home health patients

• Conduct evaluations, create treatment plans, and track patient progress

• Provide patient and family education to support recovery

• Maintain accurate documentation according to facility and regulatory standards

Requirements

• Current BLS certification

• Experience in Rehab Home Health preferred

• Ability to work independently in patient homes
