Job Overview:
Seven Oaks Rehabilitation & Healthcare Center is seeking a dedicated Occupational Therapist to support residents in a skilled nursing setting. This role focuses on helping patients regain independence and improve daily living skills through personalized therapy plans.
Key Responsibilities:
Evaluate residents and develop individualized treatment plans
Provide occupational therapy services to short-term rehab and long-term care patients
Document progress and collaborate with the interdisciplinary care team
Educate patients and caregivers on functional strategies and safety
Requirements:
Active Occupational Therapist license
BLS certification
SNF experience preferred
