Shift: Day Shift

Position Summary

Elara Caring Home Health is looking for an Occupational Therapist to deliver in-home therapy services across multiple counties in Illinois. This role centers on functional independence, patient safety, and coordinated care within the home health environment.

Core Responsibilities

• Conduct in-home occupational therapy visits and patient assessments

• Develop and manage individualized plans of care

• Oversee and support Certified Occupational Therapy Assistants (COTAs)

• Coordinate care with interdisciplinary team members

• Participate in case management activities and team meetings

• Ensure accurate and timely clinical documentation

Service Area

This position will provide coverage across the following counties:

• Montgomery

• Macoupin

• Greene

• Jersey

• Calhoun

• Madison

• St. Clair

Preferred Qualifications

• Experience with OASIS documentation preferred, not required

• Familiarity with PointCare preferred, not required

• Prior home health experience is beneficial

• Strong organizational and communication skills
