Winning Wheels – Synergy Health is seeking an Occupational Therapist to support patient rehabilitation in a structured clinical setting. This role focuses on helping individuals regain independence in daily activities through targeted therapy and consistent care.

Schedule:

• 5×8 day shifts
• 8-hour days
• Start: February

Role Responsibilities:

• Conduct patient evaluations and develop OT treatment plans
• Provide therapy focused on daily living skills and functional independence
• Track and document patient progress accurately
• Collaborate with the rehab and care team
• Support discharge planning and patient goal achievement

Requirements:

• Active Illinois OT license or eligibility
• Experience in rehab or similar clinical settings preferred
• Strong communication and documentation skills
• Ability to manage a consistent patient caseload
