About the Role
Continuum Rehab is hiring an Occupational Therapist for a skilled nursing facility in Socorro, NM. You’ll be part of a collaborative therapy team focused on helping residents regain independence and functional mobility.

What You’ll Do

• Perform OT evaluations and treatments
• Create patient-focused care plans
• Complete documentation and discharge planning
• Work closely with the rehab and care teams

What We’re Looking For

• Licensed Occupational Therapist
• SNF experience preferred
• Comfortable working full-time day shifts
