Shift: Day Shift

Role Overview
Elara Caring Home Health is seeking an Occupational Therapist to provide home-based therapy services throughout the Springfield service region. This position focuses on improving patient independence and safety while working closely with an interdisciplinary care team.

Primary Duties

• Complete in-home occupational therapy evaluations and treatment visits

• Supervise and support Certified Occupational Therapy Assistants (COTAs)

• Coordinate patient care with clinical and non-clinical team members

• Participate in team meetings and case management activities

• Maintain accurate and timely clinical documentation

Coverage Area
Services will be provided across the following counties:
Morgan, Cass, Schuyler, Scott, Brown, Adams, and Pike

System Knowledge

• OASIS documentation experience preferred, not required

• Familiarity with PointCare preferred, not required
