Schedule: Full-time, Day Shift
Hours: 5 days per week, 8:30 AM – 5:00 PM (½-hour unpaid lunch)
Guaranteed Hours: 40 per week
Contract Options: 13 weeks or 26 weeks
Start Date: ASAP

Role Summary
Colonial Vista Post Acute is seeking an Occupational Therapist to support patients across a full age range in a post-acute care setting. This role focuses on restoring function, improving independence, and supporting safe transitions through individualized therapy plans.

Key Responsibilities

• Provide occupational therapy services to patients with varying diagnoses and functional needs

• Develop and implement treatment plans aligned with patient goals

• Document evaluations, progress notes, and discharge plans per facility standards

• Collaborate with interdisciplinary care teams to ensure coordinated patient care

Position Highlights

• No weekend requirements

• Pending license candidates will be considered

• Flexible contract length (13 or 26 weeks)

• Scrubs: any solid color

Requirements

• Occupational Therapist license (active or pending)

• Ability to work with patients across all age groups

• Experience in post-acute or skilled nursing settings preferred, not required
