Position Overview:
Functional Pathways is seeking an Occupational Therapist to provide skilled rehab services at Nelson Care Center. The OT will help residents regain independence, improve daily living skills, and support overall functional outcomes in a skilled nursing setting.

Key Responsibilities:

• Deliver individualized occupational therapy treatments

• Assess and monitor resident progress

• Document evaluations, treatment sessions, and discharge plans

• Collaborate with interdisciplinary care teams to ensure coordinated care

Requirements:

• Licensed Occupational Therapist (OT)

• Experience in post-acute or skilled nursing settings preferred

• Strong documentation and communication skills
