About the Role:
Join our skilled nursing rehab team at Spanish Trails SNF, serving a resident census of 120 with a high rehab-to-home population. You will play a key role in providing comprehensive OT care, supporting residents’ functional independence, and contributing to a collaborative interdisciplinary environment.
What You’ll Do:
Provide evaluations, develop individualized OT treatment plans, deliver daily therapy sessions, manage discharges, and maintain accurate progress documentation—all focused on improving patient outcomes.
Requirements:
Licensed Occupational Therapist (OT) in New Mexico, or eligible for licensure
Prior SNF experience is strongly preferred
Strong clinical skills with effective communication and documentation abilities
