Position Overview

Functional Pathways is seeking a Physical Therapist to join the rehab team at Thorne Crest Senior Living. This role focuses on delivering personalized therapy services to help residents improve mobility, strength, and overall quality of life.

Key Responsibilities

• Conduct patient evaluations and develop individualized treatment plans
• Provide daily physical therapy sessions
• Monitor patient progress and adjust care plans as needed
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary teams to support patient outcomes

Requirements

• Active Physical Therapist license (Minnesota or eligible)
• Strong clinical and patient care skills
• Experience in senior living or rehab settings preferred
