Position Overview
Spanish Trails Skilled Nursing Facility in Albuquerque is seeking an experienced Physical Therapist to join their rehab team. This role focuses on delivering high-quality care to a diverse patient population with a strong emphasis on returning patients safely to home.
Key Responsibilities
Perform evaluations, daily treatments, and discharge planning for patients
Provide therapy to a high rehab-to-home patient population
Collaborate with interdisciplinary rehab team members
Maintain accurate documentation in accordance with facility policies and standards
Requirements
Active New Mexico PT license
Prior SNF experience strongly preferred
Ability to manage a high-volume rehab caseload
Strong communication and teamwork skills
