Position Overview:
Merrimack County Nursing Home is seeking a dedicated Physical Therapist to provide high-quality therapy services for residents in a skilled nursing environment. This role focuses on restoring mobility, improving strength, and enhancing residents’ overall functional independence.
Key Responsibilities:
Conduct physical therapy evaluations and develop individualized treatment plans
Deliver daily therapy sessions to residents
Document treatments, progress, and discharge plans accurately
Collaborate with interdisciplinary care teams to ensure comprehensive resident care
Requirements:
Licensed Physical Therapist (PT or DPT)
Experience in skilled nursing or post-acute care preferred
Strong clinical and documentation skills
