Position Details:
This role involves 6–7 home health visits per day, primarily supporting a geriatric population. Candidates must have strong OASIS experience, proficiency with Axxess charting, and the ability to manage 60–70% wound care, with the remainder focused on patient teaching. The service area covers a 20–25 mile radius of Casa Blanca. Therapist will work closely with an assigned case manager.

Requirements:
• OASIS experience
• Axxess charting proficiency
• Experience with wound care
• Arizona Fingerprint Clearance Card
• Active BLS
