Physical Therapist – Cassopolis, MI

Facility: Cass County Medical Care Facility
Schedule: 5 × 8 Hour Day Shifts (8:30 AM – 5:00 PM)
Hours: 30–40 hours/week

About the Role

This is a versatile PT position covering both Skilled Nursing and Outpatient services within the same facility. You’ll work with a wide patient population ranging from teenagers to geriatric patients, treating a mix of orthopedic, neurological, balance, and pain-related conditions.

What You’ll Do

• Perform evaluations across SNF and outpatient settings
• Develop and implement individualized treatment plans
• Treat a diverse caseload (Ortho, Neuro, Balance, Pain)
• Track patient progress and adjust therapy as needed
• Collaborate with rehab and care teams

What You Need

• Active PT License (Michigan or eligible)
• Open to new graduates
• Comfortable working across both SNF and outpatient settings

• PointClickCare (Nursing EMR)
