Job Title: Physical Therapist (PT)
Facility: Diablo Valley Post Acute
Location: Concord, CA

Schedule:

• Sunday – Thursday
• 5 × 8-hour Day Shifts
• 40 hours/week

Start Date: ASAP

Setting: Skilled Nursing Facility (SNF)

Role Focus:
Provide physical therapy services to support patient mobility, recovery, and functional independence in a post-acute care environment.

Candidate Criteria:

• SNF experience preferred
• New grads considered only with SNF clinical rotation or experience using NetHealth / PCC
• Strong documentation and care coordination skills
