Position Overview
Millennium Therapy is seeking a dedicated Physical Therapist to provide high-quality rehabilitation services in a skilled nursing environment. This role will support residents across two facilities (Cresco and New Hampton, IA – 32 miles apart), ensuring continuity of care and patient-centered treatment.
Key Responsibilities
Perform evaluations, create individualized treatment plans, and provide hands-on therapy
Treat residents in a skilled nursing facility setting
Collaborate with nursing and therapy team members to optimize patient outcomes
Track progress, document treatments, and adjust care plans as needed
Qualifications
Active Iowa PT license or Compact license at the time of application
Prior SNF experience required
Strong interpersonal and communication skills
Ability to travel between Cresco and New Hampton facilities
