About the Assignment

This role is split between two skilled nursing facilities located in Cresco, IA and New Hampton, IA (approx. 32 miles apart). Ideal for a PT who is comfortable working across locations and managing a dynamic patient caseload.

What You’ll Be Doing

• Conduct evaluations and create personalized treatment plans
• Provide hands-on therapy focused on mobility and functional recovery
• Manage patients across two SNF locations
• Track progress and adjust care plans as needed
• Collaborate with rehab and care teams

What You Need

• Active Iowa PT License (or compact) – required at submission
• Prior SNF experience is required
• Ability to travel between facilities
