Schedule: 5 × 8-hour day shifts

Coverage Area: Within a 50-mile radius (vehicle provided)

Overview

Southwestern Home Health Care is looking for a Physical Therapist to deliver in-home care across the Farmington area. This role is ideal for therapists who enjoy working independently while supporting patients in a home health setting.

Role Expectations

• Provide in-home physical therapy services to patients
• Complete evaluations, treatments, and follow-up visits
• Accurately document care using industry-standard systems
• Manage a travel-based caseload within the assigned radius

Preferred Experience

• Home Health background
• Familiarity with MatrixCare
• Experience with OASIS documentation

Requirements

• Active Physical Therapist license (New Mexico or eligible)
• Ability to travel within assigned territory
• Strong time management and independent work skills

Compensation & Flexibility

• Higher hourly rates available (up to $90/hr) based on relevant experience
• New graduates considered at a lower bill rate
• Company vehicle provided for patient visits
