Role Overview
BAYADA Home Health is seeking an experienced Physical Therapist to support patients within assisted living communities while operating under a Medicare-certified home health framework. This role blends facility-based care with home health standards, offering consistency, manageable volume, and strong work–life balance.
Key Responsibilities
Deliver skilled physical therapy services to assisted living residents
Complete and manage Medicare home health documentation, including OASIS assessments
Develop and update individualized treatment plans
Maintain accurate, compliant clinical documentation
Collaborate with interdisciplinary care teams to support patient outcomes
Experience Requirements
Minimum 1 year of Medicare-certified home health experience (required)
Proven experience with OASIS start-of-care and discharge assessments
Active Physical Therapist license (Maryland or eligible)
