Schedule: 5 × 8-hour day shifts

About the Role

Heritage Health Care Center is seeking a Physical Therapist to provide quality rehabilitation services focused on improving patient mobility and independence. This opportunity is open to both experienced therapists and strong new graduates.

Core Responsibilities

• Perform patient evaluations and create personalized treatment plans
• Deliver daily therapy sessions and monitor patient progress
• Adjust care plans based on outcomes and clinical judgment
• Maintain accurate documentation in line with facility standards

Qualifications

• Active Arizona PT license or Compact License
• BLS certification required
• New graduates welcome (adjusted bill rate applies)
• Strong clinical and communication skills
