Role Overview

Key Rehab at Mayflower Care Center is seeking a Physical Therapist to provide patient-focused rehabilitation services. This role supports residents in improving mobility, strength, and functional independence through individualized treatment plans.

Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans

• Deliver therapeutic interventions to improve mobility and functional outcomes

• Document evaluations, progress notes, and discharge summaries

• Collaborate with interdisciplinary care teams to support patient goals

Qualifications

• Active Physical Therapist license (Iowa or eligible)

• SNF/LTC experience preferred

• Strong clinical and documentation skills
