Position Overview:
Functional Pathways is seeking a Physical Therapist to provide skilled rehabilitation services at Covenant Living of Keene. This role focuses on improving residents’ mobility, strength, and overall functional independence through personalized therapy plans.
Key Responsibilities:
Deliver physical therapy treatments according to individualized care plans
Assess and monitor patient progress, adjusting interventions as needed
Document evaluations, treatment sessions, and discharge plans per facility standards
Collaborate with the interdisciplinary care team to ensure coordinated patient care
Requirements:
Licensed Physical Therapist (PT or DPT)
Skilled nursing or rehab experience preferred
Strong documentation and communication skills
