Role Overview

This is an immediate PT opportunity in a skilled nursing facility, focused on delivering hands-on rehab care to support patient mobility, strength, and independence.

What You’ll Do

• Perform patient evaluations and create treatment plans
• Provide daily therapy sessions focused on functional recovery
• Track progress and adjust care plans as needed
• Work closely with rehab and nursing teams
• Maintain accurate clinical documentation

Requirements

• Active Iowa PT License (or compact) – required at submission
• Prior SNF experience required

A structured SNF rehab environment with a steady patient flow and a strong focus on functional outcomes and patient independence.
