Overview:
Millennium Therapy is seeking a skilled Physical Therapist to join the rehab team at Keystone Nursing Care Center. The PT will provide comprehensive care to residents in a skilled nursing facility, focusing on restoring mobility, function, and independence.
Key Responsibilities:
Evaluate, plan, and implement individualized therapy programs for residents
Conduct daily treatments and monitor patient progress
Maintain accurate documentation in accordance with facility standards
Collaborate with nursing and rehabilitation staff to optimize patient outcomes
Requirements:
SNF experience required
Active Iowa license or compact license at the time of submission
Strong communication and clinical skills
Ability to work effectively with a multidisciplinary team
