Position Summary
Enhabit Home Health in La Grande is seeking a skilled Physical Therapist to provide top-quality care to patients in their homes. This is an exciting opportunity to work in a dynamic home health and hospice setting with a supportive team.
Key Responsibilities
Deliver patient-centered physical therapy services in the home setting
Perform evaluations, develop treatment plans, and provide interventions
Document patient progress accurately using HCHB
Collaborate with interdisciplinary team to optimize patient outcomes
Requirements
Valid PT license in Oregon
Home Health & Hospice experience required
Proficiency in HCHB software

Service Area: Within a 1-hour radius of La Grande
Mileage Reimbursement: $0.40–$0.48 per mile (calculated quarterly)
