Schedule:

Responsibilities:
Evaluate residents and develop personalized therapy plans
Administer daily physical therapy treatments
Track patient progress and update care plans
Collaborate with nursing and interdisciplinary teams
Maintain accurate documentation
Requirements:
Valid PT or DPT license
Experience in skilled nursing or rehab preferred
Strong communication and patient care skills
