Role Overview

Elara Caring Home Health is seeking a Physical Therapist to provide high-quality home health services in and around Litchfield, IL. This role focuses on patient-centered care delivered in the home setting, with an emphasis on evaluation, treatment, and coordinated care planning.

Key Responsibilities

• Perform physical therapy evaluations and deliver skilled treatments

• Collect and document clinical data in accordance with home health standards

• Supervise and collaborate with Physical Therapist Assistants (PTAs)

• Participate in case conferences and interdisciplinary care coordination

• Support patient progress toward functional independence and safety

Coverage Area

• Macoupin County

• Montgomery County

• Surrounding counties as needed

Preferred Skills & Experience

• Experience with OASIS documentation preferred (not required)

• Familiarity with PointCare preferred (not required)

• Home health experience is a plus

• Strong communication, organization, and documentation skills
