Shift: 5 x 8-hour Day Shifts
Duration: 13 Weeks

Overview:
Millennium Therapy is seeking a Physical Therapist to provide high-quality rehabilitation services at New Hampton Nursing & Rehab Center. This role focuses on restoring mobility, independence, and overall function for residents in a skilled nursing setting.

Key Responsibilities:

• Conduct evaluations and develop individualized therapy plans

• Provide daily treatment sessions and monitor patient progress

• Maintain accurate documentation according to facility standards

• Collaborate with nursing and rehabilitation staff to optimize care outcomes

Requirements:

• Experience in skilled nursing or long-term care preferred

• Active Iowa license or compact license

• Strong interpersonal and communication skills

• Ability to work effectively within a multidisciplinary team
