Position Overview:
All HomeCare Services is seeking a dedicated Physical Therapist (PT) to join our Home Health team. This role offers flexibility, allowing you to self-schedule visits as long as patient needs and weekly hours are met.

Key Responsibilities:

• Provide in-home physical therapy care to patients across a variety of conditions.

• Develop and implement individualized treatment plans to improve patient mobility and function.

• Accurately document patient progress and maintain compliance with care standards.

• Coordinate care with the interdisciplinary team to ensure optimal outcomes.

Requirements:

• Active Michigan PT license.

• BLS Certification.

• Prior Home Health experience preferred.

• Strong communication and organizational skills.

Schedule & Assignment:

• 40 hours per week, 8-hour days.

• Flexible scheduling available.
