Role Summary
Winning Wheels – Synergy Health is looking for a Physical Therapist to join their rehab team. You will provide skilled PT services to patients, helping them regain mobility, strength, and independence in a supportive environment.
Key Responsibilities
Evaluate patients and develop personalized treatment plans
Provide therapeutic exercises and hands-on interventions
Monitor progress and update documentation accordingly
Collaborate with OT, nursing, and other team members to ensure coordinated care
Qualifications
Valid Illinois PT license
Rehab or SNF experience preferred
Strong communication and teamwork skills
