About the Opportunity

Laurel Meadows Wellness & Rehabilitation is seeking a Physical Therapist to join its active rehab team in a skilled nursing setting. This facility has a strong rehab-to-home focus, offering a rewarding environment for therapists who enjoy helping patients regain independence and return home safely.

Your Role

• Perform patient evaluations and develop treatment plans
• Deliver daily therapy sessions focused on mobility and recovery
• Manage patient discharges and track progress outcomes
• Work closely with the rehab team to ensure quality care
• Maintain accurate and timely clinical documentation

What We’re Looking For

• Licensed PT/DPT in Missouri
• Prior SNF experience strongly preferred
• Ability to thrive in a fast-paced rehab environment
• Passion for improving patient independence and outcomes
