Role Overview
Guardian Angel Home Care is seeking a Physical Therapist to provide home health care in Santa Rosa. This travel assignment offers a steady caseload and the opportunity to work independently in patient homes, delivering therapy to a diverse population.

Key Responsibilities

• Conduct physical therapy evaluations and treatments in patients’ homes

• Develop individualized treatment plans in accordance with physician orders

• Track patient progress and document all care accurately

• Coordinate care with interdisciplinary teams as needed

Requirements

• Active Physical Therapist license (California or compact state preferred)

• Experience in home health preferred but new grads may be considered

• Reliable transportation for patient visits

Assignment Highlights
