What You’ll Be Doing

• Provide in-home physical therapy evaluations and treatments

• Develop and manage individualized care plans

• Support patients in improving mobility, strength, and functional independence

• Complete documentation per home health standards

Ideal Candidate Profile

• Licensed Physical Therapist (PT or DPT)

• Prior Home Health experience preferred

• Comfortable working independently in a community-based setting
