Role Overview:
Continuum Rehab is seeking a Physical Therapist for a skilled nursing facility in Socorro, NM. This role offers the opportunity to work alongside a strong therapy team in a well-supported rehab setting while delivering patient-focused care.

Key Responsibilities:

• Evaluate patients and develop individualized physical therapy treatment plans

• Deliver skilled therapy services to improve mobility, strength, and functional outcomes

• Document evaluations, daily treatments, and discharge planning

• Collaborate with interdisciplinary teams to support patient recovery

Requirements:

• Active Physical Therapist license (PT or DPT)

• Skilled Nursing Facility experience preferred

• Ability to work full-time day shifts
