Job Title: Physical Therapist
Location: Trinity, Texas
Facility: River Pointe of Trinity Healthcare & Rehabilitation
Shift: 5 x 8-hour day shifts (Monday – Friday, 7:30 AM – 4:00 PM)
Guaranteed Hours: 36/week
Position Overview
River Pointe of Trinity is seeking a skilled Physical Therapist to provide care in a skilled nursing and outpatient setting. The ideal candidate will deliver high-quality therapy to residents, helping them improve mobility, function, and overall wellness.
Key Responsibilities
Evaluate and treat 6–12 patients per day in SNF and outpatient settings
Administer physical therapy plans using NetHealth and PCC systems
Provide wound care as needed (training available for interested candidates)
Collaborate with interdisciplinary teams to support patient recovery
Maintain documentation and comply with facility policies and procedures
Requirements
Active Texas PT license
Previous SNF experience preferred but not required
Strong communication and patient care skills
Willingness to learn wound care protocols
