Role Overview

This is a multi-site SNF assignment requiring a PT who is comfortable floating between Hertford and Windsor, NC. The role offers consistent weekday hours with varied facility exposure.

Key Responsibilities

• Provide skilled physical therapy in SNF settings
• Perform evaluations, treatments, and discharge planning
• Support mobility, strength, and functional recovery goals
• Adapt care delivery across multiple facilities as needed

Requirements

• Active North Carolina PT License
• BLS certification
• Prior SNF experience required

Assignment Notes

• 13-week contract
• Must be open to floating between Hertford & Windsor
• 2-week notice cancellation policy applies
