Opportunity Overview

Autumn Leaves Nursing & Rehabilitation Center is hiring a Physical Therapist to support rehabilitation services across two nearby facilities in Winnfield. This role offers a consistent weekday routine with shared scheduling and billing between locations.

What Your Week Looks Like

You’ll be working standard daytime hours, treating patients, completing evaluations, and managing therapy plans. While the role is primarily weekday-based, there may be occasional Saturday evaluations, though these are rare.

• Deliver hands-on therapy and patient evaluations
• Manage treatment plans across both facilities
• Monitor progress and adjust care as needed
• Maintain timely and accurate documentation
