Role Overview
Functional Pathways is seeking a Physical Therapist Assistant to support rehabilitation services at Snyder Village in Metamora, IL. This role involves delivering hands-on therapy care to residents under the supervision of a licensed Physical Therapist in a skilled nursing setting.
Key Responsibilities
Provide physical therapy treatments as directed by the Physical Therapist
Assist residents with mobility, strength, balance, and functional activities
Monitor patient progress and report changes to the supervising PT
Ensure accurate and timely clinical documentation
Maintain patient safety and follow facility care protocols
Requirements
Active Physical Therapist Assistant (PTA) license
Skilled nursing or long-term care experience preferred
Ability to work collaboratively within a therapy team
Strong communication and documentation skills
