Position: Physical Therapist Assistant (PTA)
Facility: Winning Wheels – Synergy Health
Location: Prophetstown, Illinois
Shift: 5 x 8-hour Day Shifts

About the Role:
Join Winning Wheels at Synergy Health as a PTA supporting patient rehabilitation in a skilled therapy environment. Deliver care, assist with therapy plans, and help patients achieve their recovery goals.

Responsibilities:

• Assist Physical Therapists in carrying out treatment plans

• Provide direct patient care and therapeutic exercises

• Document patient progress accurately

• Communicate effectively with the therapy team to ensure quality care

Requirements:

• Active Illinois PTA license

• Previous SNF, outpatient, or rehab experience preferred

• Strong communication and teamwork skills

• Patient-focused mindset
