Overview:
Fiesta Park Wellness & Rehabilitation, in partnership with Continuum Rehab, is seeking a skilled SLP to provide high-quality therapy services in a skilled nursing environment. The position offers an opportunity to work within a collaborative, experienced therapy team focused on improving patient outcomes.

Key Responsibilities:

• Conduct resident evaluations, daily treatments, and discharge planning

• Develop individualized treatment plans to support functional communication and swallowing

• Maintain accurate documentation and ensure compliance with facility standards

• Collaborate with interdisciplinary care teams for coordinated patient care

Requirements:

• Licensed Speech-Language Pathologist (SLP)

• Prior SNF experience strongly preferred

• Ability to treat diverse patient populations

Assignment Highlights:

• Full-time day shift

• 13-week contract with potential for extension

• Work alongside a supportive and experienced therapy team
