Role Overview

Merrimack County Nursing Home is seeking a Speech-Language Pathologist to provide speech, language, and swallowing therapy services in a skilled nursing environment.

Key Responsibilities

• Perform speech, language, and swallowing evaluations

• Provide daily treatments and discharge planning

• Develop and update individualized care plans

• Maintain accurate and timely clinical documentation

• Collaborate with interdisciplinary care teams

Requirements

• Active SLP/CCC certification

• Prior SNF or long-term care experience preferred

• Strong communication and documentation skills
