Overview:
Continuum Rehab is seeking a skilled SLP to join La Vida Buena Healthcare in Las Vegas, NM. This role provides therapy services in a skilled nursing facility, offering the chance to work with an experienced and supportive therapy team while enjoying the scenic beauty and outdoor opportunities of northern New Mexico.

Key Responsibilities:

• Conduct evaluations, daily treatments, and discharge planning for residents

• Develop individualized therapy plans to support communication and swallowing

• Maintain accurate documentation in compliance with facility standards

• Collaborate with interdisciplinary care teams to ensure coordinated care

Requirements:

• Licensed Speech-Language Pathologist (SLP)

• Prior SNF experience preferred

• Ability to work with diverse patient populations
