Role Overview:
Functional Pathways is seeking a Speech-Language Pathologist (SLP/CCC) for a skilled nursing facility in Medina, OH. This role focuses on providing quality speech therapy services to support communication, cognition, and swallowing needs for residents in a rehab setting.

Key Responsibilities:

• Perform speech, language, cognitive, and swallowing evaluations

• Develop and implement individualized treatment plans

• Provide daily therapy treatments and complete discharge planning

• Maintain accurate and timely clinical documentation

• Collaborate with interdisciplinary care teams

Requirements:

• Active SLP license with CCC preferred

• Skilled Nursing Facility experience preferred

• Ability to work full-time day shifts
